Pharmacy software for an in-house pharmacy: from the doctor's prescription to the counter

Pharmacy software for a clinic or hospital that keeps its own medicines has one job above the rest: make sure what the doctor wrote is what leaves the shelf, from the right box, on the right bill. This guide covers what it has to record, follows one prescription in OPALEVI to the counter, and lists what OPALEVI's pharmacy does not do.

The dispensing queue of a clinic's pharmacy in OPALEVI: the Waiting tab with two prescriptions, each showing the patient's name and code, when it was issued, the prescribing doctor, one item, a Waiting badge and a Dispense button, next to tabs for dispensed and for declined and cancelled prescriptions.

Key takeaways

  • A prescription should travel as structured lines, medicine, dose, route, frequency, days and quantity, never as a note the counter has to decipher.
  • Dispensing by lot, the box that expires first going first, keeps expired stock off the shelf and tells you which batch a patient received.
  • Controlled medicines need a second person and a running balance; a register written at the moment of dispensing beats one copied up at the end of the week.
  • OPALEVI's pharmacy serves the clinic's own patients only: no retail sales over the counter, no e-prescriptions to outside pharmacies and no insurance claims.

Signs your in-house pharmacy has outgrown the notebook

A clinic pharmacy often starts as a locked cupboard, a stock sheet and a notebook for controlled medicines. It holds while one person runs it. Add a second shift, and the cracks are familiar:

  • The slip that never arrived. The patient reached the counter without it, or with someone else's.
  • Nobody knows what is waiting. The pharmacist hears about a prescription when the patient is already standing there.
  • The newer box goes first. Old stock sits at the back of the shelf until the day it is found expired.
  • Medicines leave unbilled. The box was handed over, the visit was invoiced, and the two never met.
  • The controlled book has gaps. Entries copied in later, a balance that no longer matches the cupboard.

A pharmacy inside a clinic is a link in a chain: prescriber, shelf, patient, bill. Software earns its place by keeping every link joined.

What an in-house pharmacy should record for every dispensing

Agree first on what each dispensing must leave behind, whatever the system:

The minimum record of one prescription and its dispensing
FieldWhy it matters
Medicine, dose, route, frequency, days, quantityThe counter hands over what was meant, and the patient takes it as written.
Prescriber and visitWho to ask when something is unclear, and which bill the medicines belong to.
The answer per lineDispensed, substituted with what, or declined and why: the prescriber needs to know.
Lot and expiry of what left the shelfA recall or a question about a batch is answered in minutes, not days.
Price and how it was chargedOn the bill or free of charge, decided once and visible to finance.
Witness and balance for controlled medicinesA second pair of eyes at the moment of handover, and a balance you can count against.
Reversals with a reasonA mistake put right leaves a trail instead of an erased line.

A record of what happened, not a pharmacist's judgement

Software should keep what was prescribed and what was handed over. Whether a medicine suits the patient or interacts with another stays with the prescriber and the pharmacist; OPALEVI checks neither.

Switching the pharmacy on and preparing the shelf

A new clinic starts without the pharmacy. The administrator opens Settings → Application menus, ticks Pharmacy among the Application modules and presses Save changes. That adds the Pharmacy department and turns Stock on, since every dispensing takes from the shelf. The module is offered to clinics and hospitals for people; veterinary practices do not get it.

The Application menus page in OPALEVI's settings: a list of modules with ticked boxes, among them Discharge notes, Stock, Offers, Operations, PHS and Laboratory calculator, and at the bottom Pharmacy, whose description says that switching it on adds the Pharmacy department and turns Stock on.
One tick, by the administrator. Unticking it later closes the queue and deletes nothing.

Switched off, the queue and the visit's panel close, nothing is deleted, and the controlled-substances register stays readable and exportable.

Then mark your medicines. On a stock item, the Medicine fold holds This item is a medicine the pharmacy dispenses, Controlled substance, a Sell price and VAT on sale (%). When the sell price is blank, OPALEVI charges the cost price plus the clinic's margin; with no sell price and no margin set, the medicine is free. A delivery that names its lot and expiry fills the item's Lots and batches card, and a lot under recall goes into Quarantine, where nothing dispenses it.

One prescription in OPALEVI, from the visit to the counter

A course of antibiotics in a general practice clinic with its own pharmacy, in OPALEVI for clinics; bold words are the screen's own.

  1. The doctor prescribes

On the visit, the Pharmacy panel takes up to twenty medicines. Each is picked from the clinic's medicines, with what is on the shelf beside its name, or typed under Or type the medicine, then given its Dose, Route, Frequency, days, Quantity and Instructions to the patient. Send to the pharmacy sends it, and the pharmacy's members are notified.

  2. It waits in the queue

Pharmacy opens the Dispensing queue. The Waiting tab lists open prescriptions oldest first, with the prescriber and the number of items, and refreshes itself; nurses and administrators also see Waiting at the pharmacy on Today.

  3. The counter answers each line

Dispense opens the prescription: what was asked, what is on the shelf, and the price each, which is what the save will charge, margin included. Each line's Answer is Dispense, Substitute (with Dispense instead naming the other medicine), Decline with Why it is declined, or Leave for later.

  4. The lot is chosen

Lot starts on First expiry first: OPALEVI splits the quantity across the lots in order of expiry, undated stock last. A lot can also be chosen by hand; expired, quarantined and empty lots are never offered.

  5. Charge and save

Charge is On the visit's bill or Free of charge. Record the dispensing takes the medicines off the shelf lot by lot, puts each charged line on the visit and tells the prescriber.

One prescription at the counter in OPALEVI: the patient's code, the visit date and the prescribing doctor, an items table with one capsule medicine marked To answer, and the dispensing form showing the quantity asked, the quantity on the shelf and the price each, with Dispense chosen, First expiry first as the lot, On the visit's bill as the charge, a witness field and a Record the dispensing button.
Everything the counter decides is on one form: the answer, the lot, the charge and, for a controlled medicine, the witness.

On the visit, Print prescription prints the doctor's medication lines and a table headed Sent to the pharmacy, with the pharmacy's answer beside each medicine, and Invoice this visit bills the medicines with the consultation. A patient's earlier prescriptions are on the Prescriptions tab of their file.

Until the counter starts on it, the doctor can change it with Rewrite the prescription or, giving a reason, cancel it with Withdraw the prescription.

Controlled medicines, the register, and putting a mistake right

When a line is a controlled medicine, the form asks for a Witness: an active colleague who saw the handover, never the person dispensing. Without one, nothing is saved. Each controlled line is written to the Controlled-substances register at the moment it leaves the shelf, with the balance after it.

The controlled-substances register in OPALEVI: one row for a tablet medicine with its lot number, the type Dispensed, ten out, a balance of forty, the person who recorded it with a witness beneath, and the patient in the note column, under buttons to export the ledger as CSV and to record a count.
Who handed it over, who watched, and what is left: the register answers all three on one line.
  • Counting the cupboard. Record a count takes the medicine, Counted now and a witness; a difference from the running balance is written as an adjustment, so after a count the register's balance is what is physically there.
  • The ledger for an inspection. Export the ledger (CSV) gives every entry oldest first: date, medicine, lot, type, in, out, balance, who recorded it, the witness, the patient and the note.
  • What it lists. Dispensings, voids and counts; deliveries are recorded in Stock. Keeping the register is a permission of its own, held by administrators by default.

For a wrong box or a returned medicine, Void, with a reason, puts the medicines back on the same lots and takes the charge off the visit; a controlled reversal goes into the register. A void is refused once the visit has a standing invoice; cancel the invoice first, then void.

Who prescribes, who dispenses, and the rest of the clinic

Each person has their own account and role. By default:

  • Doctors issue prescriptions from the visits they may write, and read the queue.
  • Nurses read the queue and dispense; a pharmacist's account can use this role or one you build from it.
  • Reception reads the queue and does not dispense unless you grant Dispense medicines to its role under Roles and finances.
  • Administrators do all of it, including the register and the Pharmacy settings on the queue: Margin over cost (%), VAT on medicines (%) and Mark as late after (minutes), after which a waiting prescription is flagged late.

Under Reports, a Pharmacy tab counts prescriptions issued, dispensed and declined, the average wait, and the medicines dispensed.

In a hospital, the pharmacy does not supply the wards: the Medication round records doses against the orders on the patient's Medication tab, not from the dispensing queue. The hospital surgery and ward guide covers the round, and nursing home software follows it shift by shift. For the desk that books the visit and takes the payment, read clinic reception software; OPALEVI for hospitals shows the larger setup.

A first afternoon with the pharmacy

  1. Switch it on

As administrator, tick Pharmacy under Application menus, then set the margin, the VAT on medicines and the late mark in Pharmacy settings.

  2. Stock the shelf

Mark your most dispensed items as medicines, flag the controlled ones, and record their deliveries with lot and expiry.

  3. Run one through

Prescribe on a test patient's visit, dispense it, void it, and check that the stock and the visit's bill went back to where they were.

Count before the first controlled dispensing

Record a count of each controlled medicine with a witness on day one, so the register opens on a balance you have checked yourself.

Frequently asked questions

What is pharmacy software for a clinic or hospital?

A program that links the doctor's prescription to the pharmacy counter: prescriptions arrive as structured lines, wait in a queue, are dispensed from stock by lot and charged to the right visit, and controlled medicines are recorded with a witness and a running balance.

Can OPALEVI's pharmacy sell medicines over the counter to the public?

No. It dispenses only against prescriptions issued on the clinic's own visits, so every box leaves for a patient with a visit and a bill. There is no retail till. A pharmacy that also sells to the public needs separate retail software for that side.

Can a prescription be sent to an outside pharmacy?

Not electronically: OPALEVI connects to no e-prescription service and sends no insurance claims. The patient can be handed the printed prescription, which lists the doctor's medicines and, for what went through the in-house pharmacy, the counter's answer.

Can reception dispense medicines?

Not by default. Reception can read the dispensing queue, so the desk can tell a patient whether their medicines are ready, but dispensing belongs to nurses and administrators. A clinic that wants its desk at the counter grants the Dispense medicines permission to the reception role. Finance staff hold none of the pharmacy's permissions by default.

Can we try it before paying?

Yes. Register your clinic and the free trial starts at once, with no card asked for: switch the pharmacy on and dispense a test prescription. If the trial ends without a plan, nothing is charged and your records stay readable.

How is OPALEVI priced for a clinic with a pharmacy?

Per staff seat: each staff member with an active account in the clinic counts once, and paying for a year lowers the monthly rate. Switching the pharmacy on adds nothing, and prescriptions, medicines and lots are never counted. See the pricing page.
